The Employee Contribution Fund
The Employee Contribution Fund provides support for UPMC employees working or living in Mercer and Lawrence Counties, Pennsylvania. Entirely funded through contributions made by generous employees, the Fund provides a safety net for UPMC staff facing a sudden or unexpected financial challenge. Employees are encouraged to consider supporting the Fund through a payroll deduction, and UPMC staff members in need of aid are encouraged to apply using the forms below.
Originally started at UPMC Horizon-Greenville by a group of employees, the Fund now covers employees at the UPMC Horizon-Shenango Valley and UPMC Jameson campuses, as well as other UPMC employees living or working in the region. The Fund is managed by the UPMC Horizon and Jameson Foundations, and requests for support are evaluated by a committee made up of UPMC employees.
To qualify for support from the Fund, an applicant must be a UPMC Horizon or Jameson employee, a former UPMC Horizon or Jameson employee, or an employee of UPMC in Mercer or Lawrence County, PA. Applicants do not have to be a contributor to the Fund to qualify for support. A completed application must be submitted to the Foundation or a committee member to be considered. All applications are reviewed by the Employee Contribution Fund Committee, which makes recommendations on whether the applicant qualifies for support and the amount of support which will be given. Applicants are strongly encouraged to seek assistance from UPMC’s Financial Hardship Assistance Fund, accessible through Infonet, in addition to requesting support from the Employee Contribution Fund. The Committee reserves the right to request additional information or supporting documentation for a request, and the decision of the Committee is final.

Haroldson Family
Margo and Olaf Haroldson have been with UPMC since 2001 and 2002. They are a family of seven and are very thankful for the help of the UPMC Horizon Employee Contribution Fund. Recently, their youngest child, William, was born in September of 2021. He has the congenital heart defect Tetralogy of Fallot. William had a hole in his ventricle wall with a small pulmonary valve. Payroll Deduction
To support the Fund through a UPMC payroll deduction, please download and complete the Authorization for Payroll Deduction Form and submit the completed form to the Human Resources Department at upmchorizonjamesonhr@upmc.edu.

Request Assistance
To request support from the Fund, please download and complete the Application Form. Once completed, submit the application and any supporting documentation to Kelly Haux at hauxkj@upmc.edu. Your application can also be given to any member of the Employee Contribution Fund Committee.
