The UPMC Horizon Community Health Foundation offers several opportunities to memorialize a loved one or to honor someone close to you.
The Memorial Garden at UPMC Horizon - Greenville
Memorial bricks may be ordered to be placed in the UPMC Horizon Greenville hospital memorial garden, located next to the main entrance alongside North Main Street in Greenville, Pennsylvania. Bricks may be ordered with a tax-deductible donation of $75 to the UPMC Horizon Community Health Foundation.
The Tree of Life at UPMC Horizon – Shenango Valley
Located at UPMC Horizon – Shenango Valley in Farrell, Pennsylvania, the Tree of Life in the hospital lobby presents an opportunity to honor a loved one on one of the tree’s leaves. Memorial leaves may be ordered and added to the Tree of Life with a tax-deductible donation of $75 to the UPMC Horizon Community Health Foundation.
The Memorial Garden at UPMC Horizon – Shenango Valley
Located just outside of the employee entrance on the north side of UPMC Horizon – Shenango Valley, the hospital’s memorial garden offers a beautiful opportunity to recognize a loved one through a memorial brick, inlaid in the garden courtyard. Memorial bricks may be ordered and added to the garden with a tax-deductible donation of $75 to the UPMC Horizon Community Health Foundation.

Gifts made to the UPMC Horizon Community Health Foundation to purchase a memorial item for installation on the Tree of Life or in the memorial gardens support the Foundation’s Memorial Fund. The Fund provides for maintenance of the memorial gardens and other campus beautification projects at our regional hospitals.
